How to Use One ClickUp Form Across Multiple Lists

ClickUp forms only submit to one list. Here's a workaround that uses a hidden field, URL parameters and automations to route requests to many lists.
ClickUp doesn’t natively let one form submit to multiple lists. This guide shows a simple workaround: a single form that automatically routes each submission to the right list based on who filled it out.
How the Workaround Works
Each client gets their own link to the same form. A parameter in the URL tells ClickUp who the requester is, and automations move each submission to that client’s list.
Step 1: Create a Request Tracker List
Create a hidden Request Tracker list with a dropdown custom field for the requester’s name. This list temporarily holds every form submission.
Step 2: Set Up the Form
In the form settings, add the requester field as a hidden field. Map the other fields, like request title and details, to the task properties you need. The requester value will be filled in by the form link itself.
Step 3: Add Routing Automations
In the Request Tracker list, create one automation per client. When a task is created with that client as the requester, move it to the client’s list and clear the requester value.
Step 4: Build a Unique Link for Each Client
Use a tool like Make.com to find the ID for each requester dropdown option. Add that ID to the form URL as a parameter to create a unique, pre-filled form link for each client.

Conclusion
With a hidden field, URL parameters and a few automations, one ClickUp form can feed many lists. Clients get one simple form, and every request lands in the right place.
